Our hospital is making daily efforts to provide smooth medical care. We also consider it very important to handle personal information accurately and safely. Therefore, we will make every effort to handle personal information properly as follows.
- Our hospital will formulate and implement a compliance program regarding personal information protection, and by ensuring that all staff members comply with it, we will strive for proper management of personal information.
- Our hospital will conduct educational and awareness-raising activities for all staff members regarding the importance of personal information protection, establish administrators and auditors responsible for personal information protection, and build, operate, and continue to improve an appropriate management and audit system.
- Our hospital will implement security measures to prevent unauthorized access to personal information, as well as loss, destruction, falsification, or leakage of personal information. Should any problem occur, we will promptly implement corrective measures.
- Our hospital will respect the rights of individuals concerning personal information in relation to its collection, use, and provision. We will clearly indicate the purpose of collection and the scope of use, and strive for appropriate management.
- If the person in question requests confirmation, correction, or suspension of use, our hospital will investigate and respond appropriately in accordance with separately established internal rules.
- Our hospital will comply with Japanese laws and other norms related to personal information.
- Our hospital will continuously review and improve the above activities.
Purpose of Use of Personal Information
Our hospital uses patients’ personal information for the purposes listed below.
1. For Internal Use
- Provision of medical services to patients
- Medical insurance administration
- Management and operational work related to patients, such as:
- Ward management for admission/discharge
- Accounting and finance
- Reporting of medical accidents
- Improving the quality of medical services provided to patients
- Within the hospital’s management and operations:
- Basic data for maintaining and improving medical and nursing care services and operations
- Cooperation with student training
- Case studies conducted in the hospital
For security and patient safety, cameras are installed in hospital buildings, operating rooms, and wards. Recorded images are stored for a certain period and may be used for safety management or clinical research purposes. These data are handled appropriately in accordance with our privacy policy.
To prevent patient misidentification, we may call patients by name. Patient names may also be displayed in hospital rooms.
2. For External Use
- For provision of medical services to patients, including:
- Collaboration with other hospitals, clinics, midwifery centers, pharmacies, visiting nursing stations, and nursing care providers (Within our medical corporation, medical records may be shared among hospitals/facilities as needed to strengthen medical cooperation.)
- Responses to inquiries from other medical institutions
- Seeking opinions or advice from external doctors and other specialists regarding patient care
- Outsourcing services such as specimen testing or unpaid fee collection
- Explanations of medical conditions to family members, etc.
- For medical insurance administration:
- Outsourcing of insurance-related tasks
- Submission of receipts (claims) to reviewing/payment agencies (including billing to appropriate insurers)
- Inquiries to reviewing/payment agencies or insurers
- Responses to inquiries from reviewing/payment agencies or insurers
- Notification of results to employers or contractors in the case of commissioned health checkups
- Consultation or reporting to professional organizations, insurance companies, etc. regarding medical professional liability insurance and related matters
- For hospital management and operations:
- Providing information to external audit organizations
