Double Eyelid Surgery by Incisional Method

The skin at the area where the double eyelid line will be formed is incised, and the tissues underneath—such as the orbicularis oculi muscle and the orbital septum—are treated to create the double eyelid. This method allows for a more secure double eyelid than the buried suture method and offers longer-lasting results. It is recommended for those whose double eyelid line has disappeared after the buried suture method or for those who do not want to worry about the line fading.
Also, one of the advantages of the incisional method is that it can be combined with procedures like ptosis correction, skin excision (to remove sagging), or fat removal to achieve the desired eye appearance.
Just as there are various techniques for the buried suture method, the incisional method also has different approaches depending on the doctor. The method I use involves incising the orbital septum, fixing the flipped septum to the double eyelid line, and simultaneously dissecting the area under the orbicularis oculi muscle between the double eyelid line and the eyelashes (in some cases, reducing the orbicularis oculi muscle), and fixing the skin between the double eyelid line and the eyelashes to the tarsal plate. This results in a very strong fixation.
This is because even with the incisional method, the double eyelid line can sometimes become faint.
The stronger the incisional method, the more likely it is for the crease to appear too deep and unnatural or for the downtime to be longer. However, I perform the surgery carefully to avoid these issues.
Double eyelid surgery using the incisional method has a longer downtime compared to the buried suture method. Stitches need to be removed one week later, and even after removal, swelling and bruising will remain for some time. Bruising typically disappears in about two weeks, and swelling subsides by around 90% in about one month.

Possible Symptoms and Issues After Surgery

Swelling, bruising, increased eye discharge, gritty or uncomfortable feeling in the eyes, asymmetry, double eyelid width differing from what was desired, thread being visible through the skin, noticeable knots in the thread, formation of lumps, infection, thread coming out.
