Upper Eyelid Treatments
This is an explanation of treatments for the upper eyelids, such as "upper eyelid incision", "forehead lift", and "upper blepharoptosis surgery". As we age, the sagging of the upper eyelid occurs due to two main reasons: skin laxity and weakening of the muscles.
If sagging is due to skin laxity, the double eyelid line becomes narrower, sometimes resulting in a condition called "triangle eyes". In this case, "upper eyelid incision" or "forehead lift" is selected. The "upper eyelid incision" procedure involves removing excess skin beneath the eyebrows. Since the incision is along the eyebrow line, scars are less visible and it blends naturally with the brow line. It is a relatively small procedure with short downtime and minimal swelling. During surgery, the design of the incision is carefully adjusted to avoid any change in the shape of the eyebrows. If necessary, the eyebrow shape is adjusted with permanent makeup.
The "forehead lift" involves removing skin from the hairline and lifting the forehead skin. This lifts both the forehead and the upper eyelid. This surgery not only improves forehead wrinkles but also treats muscle tension that causes eyebrow furrowing, preventing wrinkles between the eyebrows.
When sagging is not due to skin laxity but due to weakened levator palpebrae muscles, the condition is called "upper blepharoptosis". In this condition, the signal from the eye-opening muscles to the levator aponeurosis is weak, and the eyelid doesn’t open properly. In "upper blepharoptosis surgery", excess skin is removed and the levator aponeurosis is sutured firmly to the tarsal plate to strengthen the muscle's function.
If sagging is severe, upper eyelids droop and cannot be lifted, leading to the use of the forehead (frontalis) muscle to open the eyes, which may result in raised eyebrows and forehead wrinkles.
After "upper eyelid incision" or "upper blepharoptosis surgery", the eyes become easier to open, so the forehead muscles are no longer needed and the eyebrows may droop. This may make the eyes appear softer, but in some cases, people feel their face looks less lively. In such cases, a "forehead lift" can be added later to lift the eyebrows and correct the issue.
In some people, the impression of sagging eyelids is not just due to eyelid sagging, but also caused by tension and fatigue from overuse of the frontalis muscle. If you are considering upper eyelid surgery, it is also recommended to consider muscle relaxation treatments such as Botox, or forehead care before undergoing surgery.
